John Taylor Hospice provides expert palliative and end of life care for people in their own homes.
The hospice’s community teams offer care for people with a range of conditions and the care and support is ‘Taylor-made’ for each person’s individual needs.
John Taylor Hospice’s community teams include clinical nurse specialists, Hospice at Home, a Multi-Disciplinary Team and our Well-being service.
Community care includes nursing and practical help, physiotherapy, occupational therapy and help with nutrition and eating. The hospice also has a team of pharmacists and a pharmacy technician who can help with medication and alleviating side effects linked to medication.
Clinical nurse specialists in neurology, respiratory and prostate cancer also provide expert care and advice to patients and their families.
The community nurse specialists can refer patients for other John Taylor Hospice services including day hospice, the Pari Passu pain clinic, respite and in-patient stays plus the Fatigue, Anxiety and Breathlessness Clinic.
The Well-Being Team provides spiritual, emotional and psychological support through a range of services including counselling and art psychotherapy. The hospice also has a patient and family support worker who can help with issues around benefits and housing as well as signposting to other organisations.
