Our Hospice at Home team work with babies, children and families that meet the criteria for eligibility based on the four categories of life limiting or life threatening conditions:
-Conditions where curative treatment may fail
-Conditions where premature death is inevitable
-Progressive conditions without curative options
-Irreversible conditions causing severe disability
Referral for this service is via your local authority or CCG (Clinical Commissioning Groups) commissioners.

Hospice at Home
Please note hospice at home is regionally based and only available for families currently living in the Black Country.
Our Hospice at Home provides a range of clinical care and wider support in the family home comprising:
-Short breaks, emergency and end of life care
-Nursing for personal care and hygiene needs including complex nursing care
-Symptom management and pain control
-Nursing including complex care and supervision enabling other members of the family to leave the home
-Activities, play, and music and art for the child
-Support for the family to go out together – for example, accompanying them on outings
-Outings (on foot) for the child
-Sibling support including activities and outings
-Physiotherapy
-Complementary therapies – massage, reflexology, etc - for the child and family
-Support with light housework, shopping and errands
-Psychosocial support for the family and young person – including support groups, advocacy and pre and post-bereavement support

Hospice at Home support
We visit families at home and work with them to identify each family’s individual needs and the mix of services they require. A tailored package of clinical care is then developed and delivered in the family home by a multi-disciplinary team comprising registered children’s nurses, healthcare assistants, physiotherapists, family team workers and trained volunteers.
Once a child is accepted to the service a team member will visit you to undertake an initial assessment and find out what services you require. Our physiotherapist may also visit if there are any moving or handling issues.
