Learning and Physical Disability
Living with a learning or physical disability should not mean that you do not have control or choice over where and how you live your life.
Midland Heart delivers services to adults with a physical or learning disability, including people with dual diagnosis, sensory impairment, complex needs, acquired brain injury, behaviour that challenges and autism (ASD).
All our services and support are outcome focused and recognise the customer as the expert of themselves. We deliver tailored support and care packages and are keen to explore innovative ways of maximising opportunities, life experiences and choices, for example through different support models and the use of assistive technology. We work in partnership with customers, their families, carers, advocates and other organisations to support people to live the life they choose.
Midland Heart offers a range of accommodation options and support packages; residential care, shared supported living, reablement services, transitional services and independent living (support in your own home).
