A Health trainer is a personal buddy who will meet with you to discuss your health goals and help you make a plan to achieve them.

How does it work?
Our Health Trainers provide a personal one-to-one meeting. You will work with them through 6-8 sessions in which you will meet and discuss your progress. You will be supported to make small but significant changes. By working together, you will be enabled to examine and overcome barriers, giving you the tools to maintain a healthy lifestyle. They can support, encourage and motivate you to make changes by working with you to create a personal health plan. We offer local services and offer appointments at a time that fits your needs.

Who is it for?
Solihull: Anyone over the age 16 can access the service

Click here or download and print a version of our Health Trainer referral form and post it to Health Exchange, 27 Moseley Road, Avoca Court, Birmingham B12 0HJ. Alternatively, if you have a Birmingham based GP you can request for them to refer you to the program.
